Send me your form in Word, Publisher, RTF, Text or PDF format. If you do not have a form, please provide a text document of the exact phrases and questions you want on the form as well as a drawn sketch of how you want the form to appear & where the information should go. Or, find an image of a form that is in a format that you like (images.google.com). The complexity of the form will determine actual delivery date. The more complex - the longer it will take. Will also negotiate delivery date if you absolutely need it sooner.
